The Full Picture

What happens between your visit
and your statement

Five stages that normally happen behind the front desk. We put them on the table — every step, every code, every decision.

01Eligibility Check

Appointment Scheduled

We collect your insurance details before you arrive and run a real-time eligibility check — so we know your benefits before you walk in.

Insurance verified 24–48 hrs in advance
02Benefits Confirmed

Insurance Verification

We confirm your deductible status, co-pay amount, and whether any services require prior authorization — before coding begins.

Deductible · Co-pay · Prior Auth status
03CPT Codes Assigned

Medical Coding

Every service gets a CPT code. We show you the exact codes billed so there are no hidden line items when the EOB arrives.

CPT codes visible in your patient portal
04Claim Filed

Claim Submitted

Your claim goes to your insurer within 24 hours of the visit. You can track its status in real time through the patient portal.

Submitted within 24 hours · Real-time tracking
05Zero Surprises

Your Co-Pay Receipt

After your insurer processes the claim and issues an EOB, we send your final patient responsibility — the exact amount, nothing more.

Final amount matches your pre-visit quote

Plain English

The terms they
never explain

EOB. CPT. Prior auth. Coinsurance. We define every term in the billing process — the way a pharmacist explains a label, not the way a manual does.

EOB — Explanation of Benefits

Not a bill. The document your insurer mails after processing a claim — it shows what they paid, what they adjusted, and what you owe.

Think of it as a receipt from your insurance company, not a request for payment. The "Patient Responsibility" line at the bottom is what you actually owe us.
CPT Code

A 5-digit number that describes exactly what service was performed — used by every insurer in the country.

Example: 99213 = a 15-minute established patient office visit. 99395 = a preventive well-visit for ages 18–39. We show you every code before submitting.
Deductible vs. Coinsurance

Your deductible is the amount you pay before insurance kicks in. Coinsurance is the percentage you share after the deductible is met.

Example: $1,500 deductible + 20% coinsurance. Once you've paid $1,500 this year, your insurer pays 80% of covered costs and you pay 20% — until your out-of-pocket maximum.
Prior Authorization

Permission from your insurer before certain services are performed. Without it, they may deny the claim entirely.

We check prior authorization requirements before scheduling any referral, imaging, or specialty service — so you're never blindsided by a denied claim.
Eligibility Verification

Confirming your insurance is active and that our practice is in-network before your visit.

We run this automatically 24–48 hours before your appointment. If anything looks off — lapsed coverage, wrong plan year — we call you first.
Claim Submission

The electronic file we send to your insurer requesting payment for services rendered.

We submit within 24 hours of your visit. You can track the claim status — submitted, received, in review, paid — from your patient portal.
Co-Pay vs. Co-Insurance

A co-pay is a fixed dollar amount ($25) due at the visit. Co-insurance is a percentage (20%) due after the deductible.

Your insurance card lists your co-pay. We verify this before your appointment so the amount at checkout matches your card exactly.
Out-of-Network

A provider your insurance hasn't contracted with — costs you significantly more.

We are in-network with 48 plans. If a referral we make is out-of-network, we flag it before the appointment and offer alternatives.

after the statement

Real patients. Real insurance situations. Real bills they could read.

Prior Authorization

"The prior authorization check saved me from a $600 surprise. They caught it before my referral was sent — called me, offered an in-network alternative. That's the practice I want."
